As a part of the CHOC Primary Care Network, we also partner with additional primary and specialty care clinics throughout Orange County and Los Angeles County to deliver care to your child.

How to Join Our Practice as a New Patient

Step 1: Choose a Location and a Pediatrician
Our staff is dedicated to providing the best possible healthcare for our young patients.
Each of our physicians are board-certified and provide personalized care in a warm and friendly environment. Every one of our pediatricians and pediatric nurses has received extensive education, training, and experience in caring for children from infancy to teenage years and pre-adult.

Insurance Information
We accept most PPO and some HMO plans. Contact your insurance company to ensure that our practice is within your network. If you require assistance to verify coverage, let the person making the appointment know. We will confirm that we accept your insurance and verify your coverage prior to your visit.
Step 3: Prepare for your Appointment – What to Bring
On your first visit, we will begin a permanent record and family chart for you. Please arrive 20 minutes before the appointment. To expedite your visit, please fill out the forms below.

- Valid insurance card
- Co-payment (if applicable)
- Child’s immunization record
- If you have not yet received a card for new insurance, bring the insurance company name; claim address; co-payment amount; policy holder name; policy number; group number; and suffix.
- For patients with a complex or longstanding medical history, please try and bring any medical, school, or other relevant medical records

Cancellations and Tardiness
If you can’t keep an appointment please call our office to cancel as soon as possible.
If you are more than 15 minutes late for an appointment, we may need to reschedule your visit.
Immunizations
Our practice follows the immunization guidelines recommended by the American Academy of Pediatrics (AAP). Patients who are not adequately vaccinated are advised to schedule an appointment with their provider to formulate a plan to immunize their children.
Well-Visit Schedule
Well child visits are important, even when children are healthy. Your child’s pediatrician will evaluate his or her general health and development. Your child’s pediatrician will also give your child needed medical services, such as: tests, vaccinations, and medical referrals to specialists if needed.
